Pages contained within this questionnaire may vary based on information needed by the hiring agency during the onboarding process.
The Emergency page is where you provide emergency contact information.
The following elements may be displayed:
- Emergency Contacts. This is where you enter the name of an emergency contact
- First Name. This is the first name of your emergency contact.
- Last Name. This is the last name of your emergency contact.
- Relationship. This is the relationship to your emergency contact.
- Phone Number. This is the phone number of your emergency contact.
- Email Address. This is the email address of your emergency contact.

Actions. This allows you to edit or delete an entry after an emergency contact has been entered.
- Edit. This button allows you to modify the emergency contact listed. For instructions, see Editing an emergency contact.
- Delete. This button allows you to delete the emergency contact listed. For instructions, see Deleting an emergency contact.
- Add Emergency Contact. This button allows you to add an emergency contact(s). For instructions, see Adding an emergency contact.
- Enter the requested information for each physician you want to have contacted in the case of an emergency. This is where you enter the name of your physician to be contacted in the event of an emergency.
- First Name. This is the first name of your physician.
- Last Name. This is the last name of your physician.

Actions. This allows you to edit or delete an entry after an alias has been entered. The actions are:
- Edit. This button allows you to modify the alias listed. For instructions, see Editing a physician.
- Delete. This button allows you to delete the alias listed. For instructions, see Deleting a physician.
- Add Physician. This button allows you to add a physician to be contacted in the event of an emergency. For instructions, see Adding a physician.
- Preferred Hospital Name. This is the name of your preferred hospital.
- Do you have an allergic reaction to anything (food, medications, insects, etc.)?. This is where you indicate your allergic reaction information using the radio buttons.
- Previous. This button returns you to the previous screen.
- Save & Continue. This button saves any changes and takes you to the next page.
- Return to Task. This is button returns you to the Task Details page. A warning message will display, “Unsaved changes will be lost. Do you want to continue?”.
